Photo ID, insurance card if applicable, a list of current medications, and any prior dental records or X-rays you can share. Arrive 10 to 15 minutes early to complete patient intake forms.
What insurance do you accept for new patients?
We accept Medicaid and most major PPO plans including Aetna, Blue Cross Blue Shield, Cigna, Delta Dental, Guardian, Humana, MetLife, United Healthcare, and Ameritas. We also offer Cherry and CareCredit financing for uninsured patients or treatments not fully covered by insurance.
